tempSick Notes

  1. The law says you may self certify your illness for seven days before any need for an official Department of Health sick note.
  2. Your employer should provide you with a self-certification certificate (SC2) so that you can declare your illness (legally you do not need this for the first three days of your illness).
  3. If your illness continues for more than seven days, under the NHS your GP can issue an official Department of Health statement (called the Med 3 or a Med 5) as a statement of your illness.

Please note: If you require a “sick note” for an illness of fewer than seven days’ duration, a private doctor’s statement may be requested. The fee for this is £19.00, to be met by the patient or the employer.

The Receptionist will take a request for a sick note and pass it to the relevant member of staff who will check to see if it is appropriate for a GP to issue.  The GP will not always need to see you to issue this.  The triage GP is not available to discuss sick notes with patients.  The triage service is for same-day urgent medical needs only.
